WHAT you will need to register

1. CREDIT CARD - When registering, you will be provided with the option to pay 'offline' by check/cash/Venmo. You will still be required to enter a credit card for the $0.00 sale. This is a functional piece of SportsEngine and cannot be bypassed. Once you pay in person, your pending payment will be deleted.
2. HEADSHOT - Parents will need a close up/headshot/selfie of the athlete they are registering. This may ONLY include the child in the single child in the photo-NO GROUP PHOTOS.
3. BIRTH CERTIFICATE - This is only needed if you are NEW to the program. Parent signature on copy is needed.
4. MEDICAL CLEARANCE SIGNED BY DOCTOR - All athletes must submit the medical clearance form PROVIDED BY AYF (on this website). This MUST BE DATED FOR 2025. Please note: children do NOT need a physical, but must have the doctor sign the form. Doctor does NOT need to specify the date of childs last physical, as if it is from 2024 you will have to get a new physical for the current season.
5. END OF 24-25 SCHOOL YEAR REPORT CARD - This can not be submitted until the END of the school year. It MUST have all marking periods included. Please DO NOT submit report cards now.

All forms can be emailed to paperwork.redbankayf@gmail.com. Please clearly label the email with your child's first and last name and the form type(s) in the subject line. 
For example:  Austin Soden - report card and medical.

PLEASE USE THIS FORM TO HAVE YOUR CHILDS DOCTOR SIGN FOR MEDICAL CLEARANCE. ATHLETES WILL NOT BE ALLOWED TO PARTICIPATE UNTIL THIS IS TURNED IN.
